Please arrive at your scheduled time. Failing to have the complete party present within 15 minutes of the scheduled time will result in that appointment being cancelled and you will be asked to reschedule. Please come to us with all of the Required Documentation
- Evidence of U.S. Citizenship & photocopy of that evidence
- Photo Identification & photocopy of that Identification, front and back
- Passport Photos
- Form DS-11, completed but not signed (please STOP and do not write anything below the line on the first page)
- Check or Money order made payable to the U.S. Department of State for each application NO Credit Cards are accepted
- $35 Execution Fee (cash or check/money order made out to the Town of Plainville only)
- Children under the age of 18 must have their birth certificate with parents' names
- Children under 16 must be present with both parents (unless there is an extenuating circumstance communicated with form DS-3053)

If you can answer Yes to all of the questions on the front of the renewal form (DS82) then you can fill out the form, include your new picture, old passport, and payment via check or money order and mail it yourself to the address on the application. And save yourself $35 execution fee!
We only handle renewals if you cannot answer Yes to all of the questions on the front - and then you have to fill out the DS-11 and come see us with all of your documentation as listed above.
